Given the comprehensive nature of the Memory Evaluation, it is necessary to schedule lengthy appointments [3-5+ hours]. The appointment is reserved for the client. Therefore, it is not logistically feasible to schedule an alternate client on short notice.

A 72-hour notice of cancellation is required. A Cancellation Fee [15% of total fee] will be charged to the client, if cancellation occurs less than 72 hours before the scheduled appointment and without acceptable and verifiable reason.
